Chronic disease management is offered by our practice nurses this includes asthma, COPD, heart disease, diabetes, rheumatoid arthritis, stroke, high blood pressure and chronic kidney disease. Patients with these diseases should have at least an annual review and often more frequently depending on the stability of their disease.

Both our practice nurses are nurse prescribers so can prescribe medications independently at these review appointments.

Patients with high blood pressure should have their blood pressure checked at least every 6 months.
